Post-Acute Care Costs: Overcoming a Roadblock on the Path to Shared Savings
Sponsored by Salient ACO
Federal A/B
Speakers: Craigan Gray, Salient Healthcare; Louis Morgenier, Palm Beach ACO; Hymin Zucker, MD; and Amy Kotch, MHA, Healthcare Development Partners of FL, LLC

Description:
CMS has consistently listed post-acute care spending as one of the biggest issues for healthcare organizations – often consuming a significant portion of total patient spending. In conjunction with Palm Beach ACO, Salient ACO has conducted a case study on the effect of a primary care intervention on post-acute care costs and readmissions.

The speakers will lead an in-depth analysis that will address these questions:

  • How do you know if primary care physicians are involved in patients' care?
  • How do you know how much is being spent in the post-acute care setting with or without intervention?
  • How do you know how many TCMs are being billed?
  • What are your readmission rates?
  • Can you tell which physicians are doing a better job than others to contain costs?

  • Building a high performing network and guiding SNFs toward optimal patient outcomes.
  • Consistently discharging patients to preferred, high-quality providers.

Please join Erin Smith and the naviHealth team for breakfast as they highlight a specific case study involving a large ACO that used clinical decision support technology, in-market support, and advisory solutions and analytics to improve their financial and clinical outcomes.
