In a value-based care world, ACO's, are accountable for the entire patie nt journey; therefore, their long-term success and financial viability are inherently tied to the performance of their post-acute care (PAC) providers . As the ACO/PAC landscape begins to transform with many ACOs now building preferred PAC networks, there is still a lot of work to be done in creating successful care coordination efforts that align with value-based care init iatives and incorporate effective care transitions, clinical pathway standa rds of care, risk stratification, appropriate length of stays, and reduced readmissions.
Though the financial impact of this effort may seem min
imal - do we really understand the risks and costs involved if we choose to
do nothing with post-acute providers? In a recent study, conducted by the
Center for Health and Research Transformation (CHRT), it concluded that poo
rly coordinated care transitions from the hospital to other care settings c
ost an estimated $12 billion to $44 billion per year. This cost will only i
ncrease as baby boomers are estimated to double current Medicare beneficiar
ies by 2030 - demonstrating there is a true cost to inaction and at the for
efront, it is the patients that suffer.
