Please join us for a webinar on August 14th for an overview of the new My Care, My Choice website. Sponsored by The SCAN Foundation, the unique website is designed to help Californians with Medicare and Medi-Cal learn about the range of care options, including ACOs, in their local area so they can get the most from their health care coverage. By answering a few questions, the website provides a list of local care choices to fit a person's care needs and preferences.

The webinar will walk attendees through the website, including how beneficiaries may learn about ACOs in their area. The webinar will also describe ways attendees can share My Care, My Choice with beneficiaries, their family members, and community colleagues throughout the year.
