This webinar will highlight the work of Mount Sinai Health Partners and Keystone ACO's newest care management models. A high-level overview of Mount Sinai Health Partners (MSHP) Care Management Model will be provided along with discussion of their path to maximizing the interdisciplinary team when it comes to patient segmentation. Insights into MSHP's use of the ACG Johns Hopkins tool to stratify patients for care management outreach will also be provided. The Keystone ACO speaker will discuss their Care Management (CM) Playbook deployment efforts to meet needs for increasing caseload capacity and the growing complexity of its population. Their CM Playbook aims to break down the care management model into tiering levels that provide a baseline assessment for every patient, every time to apply the appropriate intervention and dosing based on the need, to move patients through the tiering with defined timelines, care gap closure and graduation criteria, and expansion of the care team members to include technology to support patients.
