Most ACOs are too small to consistently demonstrate savings in the Medicare Shared Savings Program due to the statistical variability inherent in small numbers.  In a collaborative ACO, hospitals, physician groups, and Clinically Integrated Networks (CINS) come together to form ACOs with a minimum of 100,000 lives to overcome random statistical variance and minimize costs. These ACOs feature a shared governance but are operated locally through steering committees. With enough lives and proven population health strategies, ACOs can beat the benchmark year after year.
